EACH's Diabetes Prevention Program (DPP) is recognized by the Centers for Disease Control and Prevention (CDC), and EACH is an approved Medicare Diabetes Prevention Program (MDPP) supplier through the Centers for Medicare & Medicaid Services (CMS). Led by trained lifestyle coaches, the program helps individuals at high risk of Type 2 diabetes build healthier habits through nutrition, physical activity, weight management, and ongoing support.
Classes are available in English, Chinese, Vietnamese, and other languages, with both in-person and virtual options. The year-long program includes 16 weekly core sessions followed by maintenance sessions to help participants achieve lasting lifestyle changes and reduce their risk of developing Type 2 diabetes.
This program is focused on supporting individuals who have already been diagnosed with diabetes. DSME provides education and skills to help participants manage their condition effectively. Topics include medication management, nutrition, exercise, and blood glucose monitoring. DSME is designed to empower people with diabetes to make informed self-management decisions and improve their quality of life.
Program Duration: DSME is offered as a 6-week workshop, with each session lasting 2 hours.
What You Will Learn: Participants will learn about medication management, healthy eating, the importance of physical activity, blood glucose monitoring techniques, and ways to cope with the emotional aspects of diabetes.
Who Should Sign Up: Individuals who have been diagnosed with type 1 or type 2 diabetes and are looking for guidance on how to better manage their condition.
This program is for individuals at high risk of developing type 2 diabetes. DPP emphasizes lifestyle changes aimed at preventing the onset of diabetes. Participants learn about weight loss, dietary improvements, and increasing physical activity. The DPP creates a supportive environment to help individuals make sustainable changes that reduce their risk of developing diabetes.
Program Duration: DPP is a 12-month program with 16 weekly sessions at the beginning and monthly maintenance sessions after that. Each session lasts 1 hour.
