EACH is proud to announce that it has been awarded a multi-year Community Health Grant (2019–2022) from Beth Israel Deaconess Hospital–Milton (BID–Milton), a regional community hospital in Milton, Massachusetts, that provides comprehensive inpatient and outpatient healthcare services.
This funding supports the development and implementation of a culturally responsive Chronic Disease Management Program to reduce health disparities and address risk factors affecting Asian American communities. Through community education, outreach, and preventive health initiatives, the program strengthens access to care and promotes long-term wellness. We are deeply grateful to BID–Milton for its partnership and generous support of EACH’s mission to advance equitable health outcomes.
In collaboration with the Castle Square Tenants Organization, located at 476 Tremont Street, Boston, MA, EACH delivered a series of Prevention and Wellness Programs for residents from Q2 through Q3 of 2019. This initiative was designed to provide culturally responsive health education tailored to the community's needs.
The program focused on increasing awareness of chronic diseases prevalent among Asian American populations, promoting disease prevention strategies, and offering guidance on navigating government health plans and public benefits programs. Through education and outreach, EACH aimed to empower residents with the knowledge and resources needed to make informed health decisions and improve overall well-being.
